Infusion Logs

The infusion log is used as a tool for your members to provide information to prescribing physicians, our pharmacy and insurance plans. The information recorded on the infusion log enables you to monitor your members' treatment and make adjustments as needed, and can help determine whether certain activities increase the risk for bleeding.

The recommended information that your members should capture on their logs to maximize the effectiveness of this tool should include:

  • Date of the infusion
  • Factor product used
  • Dose administered
  • Lot number from the factor
  • Expiration date
  • Reason the factor was prescribed (prophylaxis, treating a bleed, where it was located)

Most private, state and federal insurance carriers now require a copy of this log to confirm that patients are using their factor as prescribed.
